  4. Certified true copy (Blue)

    • T.O.R or Transcript of Record (if college/vocational graduate or college/vocational level)
    • FORM 137 (if high school/elementary graduate or high school/elementary level)
    • DIPLOMA

  5. Performance rating in the last rating period (if applicable)
  6. Relevant trainings (if necessary)
  7. Committee/Community Involvement Certificate or Office Memo (Tree Planting, Membership in TWG etc.)
