 REQUEST FOR NEW SERVICE CONNECTION  
 
Proof of ownership [ one (1) Photocopy ] Any of the Following:

 I. For applicants who own the lot
 - Lot Title
 - Building Permit
 - Deed of Sale or Deed Donation
 - Certificate from housing office or housing developer prescribing therein the vested right to the property
 - Tax Declaration with TCT number
 - Valid identification card
 - Other proof of ownership

  II. For applicants duly authorized by the lot owner
 - Lease contract  - Affidavit of Administratorship / Affidavit of Heirs
 - Board Resolution, SEC Certificate, Special Power of Attorney
 - Valid identification card (as may be applicable)

 III. For applicants occupying public land
 - Barangay Certificate and Affidavit of Undertaking
 - Valid identification card (secure form from the Customer Service Assistant)

 IV. For applicants who are informal settlers on private titled land
 - Certificate from the Homeowners’ Association/Duly accredited Settler’s Assn
 - Valid identification card (secure form from the Customer Service Assistant)

 V. Additional requirements for buildings two storey and above
 - As-built Plan or Site Development Plan with vicinity map
 - Isometric with pipe sizes
 - Plumbing Lay-out Plan signed and sealed by a Master Plumber

 REQUEST FOR RECONNECTION

 Reconnection may be re-opening or reinstallation. Reopening is done when the water meter is plugged during disconnection.  Reinstallation is when the water meter on site was already dismounted.   

 Requirements:
 - Payment of the outstanding balance and reconnection fee
 - Valid ID of account holder
 - If authorized representative, valid ID’s of account holder and authorized representative and authorization

 REQUEST FOR DISCONNECTION

 When requesting for disconnection, the water meter shall be immediately dismounted.

 Requirements:
 - Payment of the outstanding balance
 - Valid ID of account holder
 - If authorized representative, valid ID’s of account holder and authorized representative and authorization
 - Payment of Marginal Bill


 REQUEST FOR INVESTIGATION DUE TO HIGH CONSUMPTION

 - High consumption occurs when the bill for the month reached to more than the normal monthly billing
 - The cause of high consumption shall be determined during the investigation


 REQUEST FOR LEAK ADJUSTMENT

 This special privilege shall be given to accounts with high consumption due to waterline leaks wherein the customer can avail  adjustment of one month bill during the period when the leaking occurred

 This shall be granted only once

 Requirements:
 - Valid ID of account holder
 - Certificate of Repair
 - If authorized representative, valid ID’s of account holder and authorized representative and authorization


 REQUEST FOR TRANSFER METER

 Transfer of Meter shall not be allowed if there is a change in the location of the house or lot to be supplied

 Requirements:
 - Valid ID of account holder
 - Transfer meter fee:
            > For location to be transferred has existing water meters - Php 200.00
            > For location to be transferred has no existing water meters – Php 1,500.00
 - If authorized representative, valid ID’s of account holder and authorized representative and authorization


 REQUEST FOR STOLEN METER

 Requirements:
 - Valid ID of account holder
 - Police Report
 - Stolen Meter Fee of P1,200.00
 - If authorized representative, valid ID’s of account holder and authorized representative and authorization


 REQUEST FOR CHANGE OF ACCOUNT NAME AND OTHER INFORMATION

 I. In-case of Sale of Property:
           > Letter of request
           > Proof of Ownership
                       a. For Private Ownership
                                  1. Deed of Sale
                                  2. Lot Title
                       b. For Informal Settlers
                                  1. Barangay Certification & Affidavit of Undertaking
            > Any Valid Identification Card (ID)

 II. In-case of Waiver:
            > Letter of request with Waiver
            > Valid Identification Card (ID) of waivor
            > Valid Identification Card (ID) of waivee

 III. In-case of Death:
            > Death Certificate
            > Valid Identification Card (ID) of Authorized Heir
            > Affidavit of Heirship

 IV.   In-case of Marriage:
            > Marriage Certificate/Contract
            > Valid Identification Card (ID)

 Note:
 - For Authorized Representative:
   Bring all requirements under which cases they belong and his own Valid ID and authorization letter.

 - For Corporate Representative:
   Bring all requirements under which cases they belong, authorization covered by Board Resolution confirmed by Board   Secretary.
